St. Leonard’s church—also known as The Actors’ church—is the burial place of many prominent early modern actors. The Burbages (James Burbage and his sons Richard Burbage and Cuthbert Burbage), Richard Cowley, William Sly, and many others are buried there (ShaLT).
For a detailed history of St Leonard’s church, see James Bird’s history of Shoreditch on British History Online. For a modern map marking where the church once stood, a guide for a walking tour to the church, and a gallery of photos, see the Shakespearean London Theatres (ShaLT) article on St. Leonards.
